Plan4Life benefits table
There are three levels of cover to choose from. The table below shows the maximum benefit payable depending on the type of diagnosis.
Payment is by individual monthly direct debit and the premium payable will depend on whether the policyholder is male or female and their age. For policyholders who smoke, the payment that can be claimed are half those shown in the benefit table below.

Premiums will increase with age in accordance with the premium table above and will be collected at the new rate on the first payment date after the birthday on which you move into a new age band. We will write notifying you of each premium increase.
